I did almost everything different from Ryu1999 except for the tunnels part, I do think to have everybody survive you have to do the loyalty missions and upgrade your ship and crew upgrades. I did everything differently except I made Zaeed escort. Which when I think about it wasn't a smart choice. And to make sure that no one got the wrong idea when I was agreeing with dark10x's posts from way back - I too think that Mass Effect 2 is so much better in practically every way over Mass Effect 1 - even though there are a couple things I miss from the first one.
